Trigger tools and algorithms in the management of chronically ill home care patients

Abstract In the healthcare domain, triggers are defined as clues to identify adverse events. Trigger tools are typically employed for the early identification of patients at risk of adverse events and to measure the overall level of harm from medical care in a health care organization. The application of trigger tools is particularly critical in the home care domain owing to the heterogeneity of medical records and the presence of multiple caregivers with cross-communication issues. Focusing on chronically ill patients suffering critical fall events and preventable repeated hospitalization, the goal of this project is to streamline a workable strategy for home care management by boosting the intelligence of trigger tools. Specifically, we plan to explore the application of trigger tools that leverage electronic health record data to improve the detection rate of preventable adverse events (such as diagnostic errors).
