Today more than ever, taking care of a person means knowing how to work together: institutions, services, professionals and the community. The network thus becomes much more than a concept: a concrete practice, made up of relationships, trust, exchanges and shared vision.
In the world of case management, no one really works alone. The case manager operates in a complex context, where multidisciplinarity, interinstitutional collaboration, and active involvement of the person are the real strengths. However, there is no shortage of challenges: difficult coordination, fragmented communication, and unclear roles are some of the elements that can slow down or even compromise caretaking.
The meeting, now in its fifth year, will feature dynamic discussion and interaction through a stimulating hands-on activity designed to foster the exchange of experiences.

Horizons Case Management is a spontaneous interest group born from synergies and interests developed over the years, particularly in the field of CAS Case Management training. These experiences have materialized into a path of aggregation and development of the application of the Case management methodology in Canton Ticino in different sectors: social, health, insurance and services.

Three experts discuss key issues, stimulating reflections. - 17.50 Interactive Open Space
People attending will be involved in a practical and dynamic activity, divided into thematic groups to share ideas, experiences and solutions. An opportunity to get involved and experience networking firsthand. - 18.50 Summary of the work done in the open space
Restatement of the main insights that emerged during the Open Space, with space for questions and collective discussion. - 19.15 Focus on key issues and networking
Shared identification of the most relevant issues, followed by informal networking time with complimentary appetizers.

- Andrea Banfi, Sociologist and pedagogist, DEASS SUPSI lecturer.
- Sabrina Revolon, Nurse expert in home care, DEASS SUPSI professional lecturer in charge of family and community nursing area.
